What counts as verification?

A confident answer and a successful tool response are useful signals, but they do not by themselves establish the outcome.

Choose the check first

Write down what success means before an action. For a calculation, recompute from the inputs. For a file, open and inspect the result. For an external action, consult the authoritative system record.

Keep evidence connected

Preserve the task, input sources, approved scope, attempted action, and check result together. If the evidence is incomplete, identify the gap instead of marking the outcome verified.

Handle failure openly

A failed check should stop the completion claim. Correct the action or evidence, repeat the relevant check, and state what remains uncertain. Consequential decisions still require human review.
