Monitoring, severity definitions, incident ownership, communication channels, and recovery exercises must be established for the actual deployed service.
When something fails
Do not treat a missing response or partially completed action as success. Preserve the original task and known evidence, check the authoritative result, and avoid repeating a consequential action without knowing whether it already happened.
